FITNESS CLASS WAIVER & RELEASE OF LIABILITY (NEW YORK)
I understand that participation in fitness and exercise activities involves physical movement and exertion and carries inherent risks, including but not limited to muscle strain, joint injury, falls, dizziness, shortness of breath, cardiovascular stress, or other physical injury. I voluntarily choose to participate in a fitness class or workout offered in a studio setting in New York State. These activities may include, but are not limited to, strength training, conditioning, mobility work, stretching, Pilates-based exercise, or other forms of physical fitness.

I acknowledge that I am physically able to participate and that I have no medical condition, injury, or limitation that would prevent safe participation, or that I have consulted with a qualified medical professional prior to participating.
I understand that it is my responsibility to listen to my body, modify movements as needed, and immediately stop participation if I experience pain, dizziness, discomfort, or any concerning symptoms. I agree to follow all instructions provided by the instructor. In consideration of being permitted to participate in this class, for which a participation fee may be charged, I hereby release, waive, discharge, and hold harmless the instructor(s), studio, property owner, organizers,
employees, contractors, and affiliates from any and all claims, demands, causes of action, or liabilities arising out of or related to any injury, illness, or damages that may occur as a result of
participation, whether caused by negligence or otherwise, to the fullest extent permitted under New York law.

I acknowledge that participation is voluntary and undertaken entirely at my own risk. I grant permission for emergency medical treatment to be administered if deemed necessary, and

I agree to be financially responsible for any medical expenses incurred.

I understand that PAID LTV fitness class are NOT being recorded, photographed, or broadcast, and NO media or likeness rights are granted or required as part of participation.

I understand that FREE LTV Fitness Studio classes will be recorded and will be broadcast on all our platforms. By attending these free classes you are consenting to being filmed, photographed, and/or otherwise recorded and to the release, publication, exhibition, or reproduction of any and all
recorded media you may appear in for any purpose whatsoever. in connection with Local Tv Inc. and its initiatives, including but not limited to use on the LTV cable channel, website, newsletter,
YouTube channel, and/or other social medias. We’re grateful for your participation and look forward to sharing this event with our community. Enjoy the show!
